How to set up Patient Portal: Please provide us with your email and mobile number. You will receive an email or text from Practice Fusion as we will invite you to the patient portal. You will be able to schedule appointments and check your records. The instructions to set up the portal are below.
Select “Access your records” in the email or text you received from Practice Fusion.
Select “Enter PIN received from our office” and press continue.
Enter patient’s date of birth (MM/DD/YYYY) and PIN.
Click “Verify identity.” Once you have been verified, select “Create new account” and continue.
